Health Coach
Through a collaborative agreement with Rossendale Leisure Trust the two Rossendale PCNs (Rossendale East PCN and Rossendale West PCN) are to appoint an additional Health Coach.
Rossendale Leisure Trust are co-ordinating the recruitment for this new position and the selected candidate will work as part of their network’s multidisciplinary teams and in particular working closely with the Rossendale PCN & the 9 practices across Rossendale.
